Healthy Places Index
The Healthy Places Index (HPI) is widely used across California to measure to health equity across communities, as well and identify vulnerable places. HPI finds equity issues facing communities to be policy, systems and environmental issues, instead of individual behaviors and abilities.
The HPI score is developed using 23 indicators that predict life expectancy. Improving any of these indicators, especially those with the most weight, will lead to increases in quality and length of life for residents.

HPI Scores
HPI scores are calculated for almost every Census Tract in California. The distribution of scores is often grouped into quartiles (25% of all tracts) to create an easy scale for assessing the needs of the population. Quartile 1 represents the lowest scores (highest need), while Quartile 4 tracts have HPI scores in the top 25% of all tracts, indicating that those populations have more economic and social advantages.
Whole Person Health Score
In an effort to provide equitable services and improve outcomes for our diverse population, The Riverside University Health System’s (RUHS) Whole Person Health Score considers numerous factors that can affect a person’s wellbeing, going beyond the immediate issue that brought them to the doctor's office.
The Whole Person Health Score measures:
- Physical Health
- Emotional Health
- Resource Utilization
- Socioeconomics
- Ownership / Activation
- Nutrition / Lifestyle
Answers to each question are converted into a final score that can be tracked as the patient collaborates with their healthcare team to improve their overall health and wellness. The assessment provides insight into the needs of patients and gives them the tools they need to encourage healthy behaviors.
